個人資料蒐集同意書
  1. 台灣護理學會(以下稱本活動單位)委託優事資訊股份有限公司提供線上投稿相關服務，並確保帳號註冊人之利益，將遵循個人資料保護法及相關法令之規定，蒐集、處理及利用註冊者所提供之個人資料。
  2. 帳號註冊人就其個人資料得依個人資料保護法第三條之規定，行使查詢或請求閱覽、請求製給複製本、請求補充或更正、請求停止蒐集、處理或利用、請求刪除之權利。
  3. 帳號註冊人得自由選擇是否提供個人資料或行使個人資料保護法第三條所定之權利，惟帳號註冊人提供資料不足或有其他冒用、盜用、不實之情形，將無法完成程序並喪失享有活動後續服務之權益。
  4. 在帳號註冊人就其個人資料依個人資料保護法第三條之規定，請求停止蒐集、處理或利用或請求刪除前，本活動單位得依循個人資料保護法及相關法令之規定，於個人資料提供之範圍與目的內使用該等個人資料。
Personal Data Collection Agreement
  1. Taiwan Nurses Association (hereinafter as the activity unit) entrusts YourSaaS Information Co., Ltd. to provide online submission and related services, and to ensure your rights and interests, will comply with the Personal Information Protection Act while collecting, processing and using your provided personal information.
  2. In accordance to the Personal Information Protection Act Article 3, Applicants are entitled to inquiry and request for a review of your provided personal information, request to make duplications of your provided personal information, request to supplement or correct your provided personal information, to discontinue the seminar’s collection, processing or use of your provided personal information and to delete your provided personal information.
  3. Applicants are free to decide whether or not to provide the relevant personal data. If you provide incorrect, untrue or incomplete information, will not be processed and lose the entitlement to subsequent services connected with the activity.
  4. Please be rest assured that your personal information is to be used by the activity in the preparation period, during the activity for communication and data sending purposes only.
